It applies to all Personal Information processed by us in connection with or as part of providing our Services (defined below) or engaging in written or electronic communications with you. 

This Policy does not describe how we collect or use your Protected Health Information (as defined by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”)), which is covered by our Notice of HIPAA Privacy Practices.

We collect Personal Information when you use our Services, create an account with us or provide Personal Information to us. “Personal Information” is any information that identifies, relates to, describes, is capable of being associated with, or could reasonably be linked, directly or indirectly with a particular individual, including, but not limited to, a first and last name, unique identifier, email address, a home, postal or other physical address, and phone number. Information You Provide to Us
We collect information you give us when you use our Services, including when you book an appointment, when you participate in surveys or promotional activities, give us feedback, or when you otherwise choose to provide your information to us.

  1. When you book an appointment we may collect your name, address, phone number, email address, birth date, sex, payment information, gender identification, and a medical intake form. 

  2. From time to time, we may invite you to participate in surveys or promotional activities. If you choose to participate, we may collect contact details like your name and email to follow up on feedback or, if applicable, notify contest winners.

Please note, we use third-party services for our Electronic Health Record (“EHR”) and Patient Portal, video conferencing, and data analytics. We will not store or collect your payment card details, or lot-in information directly. That information is provided directly to and managed by our third-party processors whose use of your personal data is governed by their privacy policies. Currently, the EHR Patient Portal we work with is:

Communications from You
When you use our Services (for example on our Website), complete electronic forms (such as the “Contact Me” form on our Website), or contact us by email, phone, or text, we may automatically collect and store certain information about you and the activity you engaged in, for example: your name and contact information; information that you voluntarily provide to us; the nature of your communication; the purpose of the interaction, and the action we took in response to your inquiry or request.

Information Related to Your Use of the Services
We may automatically collect information about your use of the Services (we refer to this information as “Usage Data”), including information sent by your device. For example, we may collect:

  1. Device information, such as your hardware model, IP address (the Internet address of your computer), unique device identifiers, and other information such as your browser type and operating system.

  2. Website usage information regarding customer traffic patterns and website usage. This may include the web page that you were visiting before accessing our Website , the pages or features of our Website you browsed to inform us which part of our Website you visit and how much time you spend there.

We collect certain information that your mobile devices send when you use our Services, such as the unique identifier, user settings and the operating system of your device, as well as information about your use of our Services on your mobile device.

Location Information
We do not collect precise information (e.g., GPS data; latitude and longitude) concerning the location from which you access our Services, but we may collect information about your general location by converting your IP address into a rough geo-location. We may also access your mobile device’s GPS coordinates or course location, but only if you have previously agreed that we can collect this information by allowing the sharing of your location information. If you do not want us to have your location information, you may disable the location sharing feature on your device or browser.

We may receive your Personal Information from our business affiliates and partners in connection with one or more business purposes, including to make our Services available to you.

Please note that we use a third-party analytics service for data analysis. We will not store or collect activity details, browser/app/device information, or location information directly. That information is provided directly to and managed by our third-party processors whose use of your personal data is governed by their Privacy Policy.

We use marketing to grow our community and inform you of new products and services, including those offered with our partners or affiliates. We may use your contact information or information about your interactions with our Services to send you marketing communications, update you about our offerings, provide informational content, and deliver targeted marketing. Periodically, we may send newsletters and emails that directly promote our Services and that we believe may interest you.  You will have the opportunity to “opt-out” of marketing communications through the opt-out button on the newsletters and emails. However, we will still need to send you certain administrative and transactional communications about the Services, including updates to our Terms of Use, this Policy, or billing.

When you contact us with questions, feedback, disputes, or issues, we collect information to address your needs and support your use of the Services.

Although no website can guarantee security, we take measures to keep you secure while using our Services, including processing your device information, log-in information, activity information, and other relevant details to proactively manage privacy and security risks.   1. Children and Privacy

The Company is committed to protecting the privacy of children who may use our Services under the supervision of a parent or guardian. This section explains our practices regarding the collection, disclosure, and consent requirements for information collected from children under the age of 13 or 16 (herein referred to as “child” or “children”), in accordance with the U.S. Children’s Online Privacy Protection Act (“COPPA”) and other applicable state privacy laws.

Our Services may be used to facilitate healthcare for children, but children under 18 cannot directly register for an account on our Patient Portal. A parent or guardian must create an account on behalf of a child and attest to their legal authority to do so. If your child uses the account directly, either with or without your permission, we may collect information directly from the child. If you prefer your child not interact with us online, please do not share account credentials with them. Certain state privacy laws may allow a child to obtain specific types of healthcare services independently of a parent or guardian.

Cookies
A "cookie" is a small text file that may be transferred to your computer's hard drive to personalize our Services for you and collect information regarding usage of our Services. Each computer is assigned a different cookie that contains a random, unique identifier. Our Services may use two different types of cookies: a “session” cookie, which is required to track a user session, for example, and which expires shortly after the session ends, and a “persistent” cookie, used to track unique visits to the Website, as well as how the user arrived at the Website (for example, through an email link or from a referral link). A “web beacon,” “clear GIF,” “web bug,” or “pixel tag” is a tiny graphic file with a unique identifier that is similar in function to a cookie, but would allow us to count the number of users that have visited certain pages or screens of our websites, and to help determine the effectiveness of promotional or advertising campaigns. In contrast to cookies, which may be stored on your computer's hard drive, web beacons are typically embedded invisibly on pages or screens. We may use web beacons in providing the Services.

Do Not Track Requests
We do not honor Do Not Track (“DNT”) requests. To determine whether any of our third-party service providers honor DNT requests, please read their respective privacy policies.
